Summary

Temporal lobe epilepsy (TLE) is associated with atrophy in the temporal pole (TP), particularly the white and gray matter. This TP atrophy often indicates the seizure origin and may share a common cause with white matter signal changes.

Background:

  • Temporal lobe epilepsy (TLE) is a common neurological disorder.
  • Anterior temporal white matter increased T2 signal is a known MRI finding in TLE.
  • The underlying pathophysiology of this white matter signal change and its relation to temporal pole structures is not fully understood.

Purpose of the Study:

  • To quantify morphologic changes in temporopolar structures in TLE patients.
  • To investigate the relationship between temporal pole (TP) volume and MRI findings, including anterior temporal white matter increased T2 signal.
  • To understand the pathophysiology of TLE.

Main Methods:

  • MRI scans of 30 TLE patients and 30 controls were analyzed.
  • Three-dimensional T1 MR images were used for semiautomatic measurement of TP gray and white matter volumes.

  • Statistical analyses assessed associations between TP volume, T2 signal, hippocampal sclerosis, and clinical variables.

    • Patients with TLE showed reduced ipsilateral TP white and gray matter volumes.
    • Abnormal TP volume correctly lateralized the epileptogenic zone in 72% of patients.
    • Increased T2-weighted signal in anterior temporal white matter correlated with greater TP volume asymmetry.

    Conclusions:

    • The temporal pole is frequently atrophic in patients with refractory TLE, ipsilateral to seizure onset.
    • TP atrophy and increased white matter T2 signal may stem from a shared pathological process.
    • TP volume measurement is a valuable tool for lateralizing the epileptogenic zone in TLE.
