Background:

Primary cerebral actinomycosis is an exceptionally rare fungal infection of the brain.
Actinomycosis typically presents as a chronic, granulomatous disease, often involving abscess formation.

Observation:

A unique case of primary cerebral actinomycosis without abscess formation is described.
Pathological examination revealed two distinct lesion types within the same granulomatous mass.

Findings:

The co-occurrence of different lesion morphologies in a single granulomatous mass is the most significant pathological finding.
The differential diagnosis includes co-infection with Actinomyces israelii and Actinobacillus actinomycetemcomitans or Actinobacillus lignieresi.

Implications:

This case expands the understanding of cerebral actinomycosis presentations.
It underscores the importance of considering mixed infections in rare neurological fungal diseases.
Further research may clarify the pathogenic mechanisms of dual Actinomyces and Actinobacillus infections in the central nervous system.
