Summary

This study reports a rare case of myopericytoma, a benign tumor, found entirely within a blood vessel. This finding expands the known locations for this type of perivascular myoid cell tumor.

Proliferative Nodule With Unusual Features in a Giant Congenital Melanocytic Nevus.

Background:

  • Myopericytoma is a benign tumor originating from perivascular myoid cells (myopericytes).
  • Typically found in the skin or subcutaneous tissue of extremities in adults.

Observation:

  • A 54-year-old male presented with a painful, slow-growing thigh nodule.
  • Histologic examination revealed the lesion was entirely intravascular, located within a vein lumen.

Findings:

  • The tumor exhibited characteristic myopericytoma features: spindle cells in a concentric pattern around vessels.
  • Immunohistochemistry showed positivity for smooth muscle actin and CD34, with negativity for other markers.
  • Some areas showed fascicles of spindle cells within the lesional vessels, resembling myofibroma.

Implications:

  • This case demonstrates that myopericytoma can occur intravascularly.
  • Highlights the importance of considering unusual locations for myopericytoma diagnosis.
  • Contributes to understanding the spectrum of benign vascular and perivascular tumors.
