Summary

Dermatomyositis is linked to cancer in 15% of cases, often with severe skin symptoms. Treating the tumor first improved response to immunosuppressive therapy for dermatomyositis.

Background:

  • Polymyositis and dermatomyositis are idiopathic inflammatory myopathies.
  • The association between these conditions and malignancy requires further investigation.

Observation:

  • A retrospective analysis of 214 patients with polymyositis/dermatomyositis was conducted.
  • Malignancy was identified in 9 out of 60 dermatomyositis patients (15%).
  • No tumors were observed in 154 polymyositis patients.

Findings:

  • Dermatomyositis patients with neoplasia presented with severe cutaneous manifestations.
  • Simultaneous diagnosis of dermatomyositis and cancer required more aggressive treatment.
  • Tumor treatment (surgery, radiotherapy) improved the response of skin and muscle symptoms to immunosuppressive therapy.

  • Dermatomyositis patients, especially those with significant skin involvement, should be screened for underlying malignancies.
  • Early cancer detection and treatment may improve outcomes for patients with co-occurring dermatomyositis.
  • This study highlights the importance of a multidisciplinary approach in managing dermatomyositis associated with cancer.
