    Summary

    Patients with Mobitz type I atrioventricular block and bundle branch block require electrophysiologic studies. This helps diagnose intermittent complete heart block and guides treatment for syncope.

    Background:

    • Syncope can be caused by various cardiac arrhythmias.
    • Atrioventricular (AV) block, particularly Mobitz type I, can present with conduction abnormalities.

    Observation:

    • A patient presented with recurrent syncope and electrocardiographic findings of Wenckebach type second-degree AV block and left bundle branch block.
    • Electrophysiologic studies, specifically His bundle recordings, were performed.

    Findings:

    • The His bundle recordings revealed Mobitz type I AV block localized within the ventricular specialized conduction system.
    • This finding was unusual and suggested intermittent complete heart block.
