Summary

Preoperative embolization using estrogen alcohol and polyvinyl acetate effectively treats complex arteriovenous malformations (AVMs), expanding surgical options. Early surgery is advised if hemodynamic changes occur post-embolization.

Area of Science:

  • Neurosurgery
  • Interventional Radiology

Background:

  • Arteriovenous malformations (AVMs) present significant surgical challenges.
  • Preoperative embolization is a strategy to reduce AVM size and vascularity before surgical excision.

Purpose of the Study:

  • To evaluate the efficacy and safety of preoperative embolization with estrogen alcohol and polyvinyl acetate for treating Spetzler-Martin grade I-V AVMs.
  • To assess the impact of embolization on surgical indications and outcomes for complex AVMs.

Main Methods:

  • Retrospective review of 37 patients with AVMs treated between 1991 and 1997.
  • Utilized estrogen alcohol and polyvinyl acetate for 69 embolization procedures.
  • Surgical resection guided by intraoperative digital subtraction angiography.

Related Experiment Videos

Main Results:

  • AVM grades improved in 10 patients post-embolization.
  • Three hemorrhages occurred post-embolization, likely due to hemodynamic stress or drainer occlusion.
  • Complete one-stage AVM resection was achieved without major neurological deficits, though one case of normal perfusion pressure breakthrough occurred.

Conclusions:

  • Preoperative staged embolization with estrogen alcohol and polyvinyl acetate expands surgical indications for difficult AVMs.
  • Complete resection of complex AVMs is achievable with this combined approach.
  • Prompt surgical intervention is recommended upon observing hemodynamic changes after embolization.
