Summary

Thalidomide treatment can cause severe lung toxicity, a previously unreported side effect. This case highlights the need to consider pulmonary complications when patients on thalidomide present with respiratory symptoms.

Background:

  • Thalidomide is a known teratogen with established side effect profile.

Observation:

  • A 65-year-old male with multiple myeloma developed acute coughing, dyspnea, and malaise on thalidomide.
  • Chest imaging revealed an interstitial and alveolar pattern, with arterial blood gases indicating partial respiratory insufficiency.

Findings:

  • Microbiology results were negative for common respiratory pathogens.
  • Symptoms resolved following thalidomide withdrawal and supportive care with oxygen and corticosteroids.

Implications:

  • Severe lung toxicity should be recognized as a potential adverse effect of thalidomide.
  • This finding necessitates vigilant monitoring for respiratory symptoms in patients receiving thalidomide.
