Area of Science:

  • Hepatology
  • Transplantation Surgery
  • Gastroenterology

Background:

  • Hepatolithiasis (intrahepatic stones) is prevalent in Asia but rare in the West.
  • Advances in surgical techniques have decreased recurrent stones and liver damage.
  • However, significant mortality persists due to liver failure, varices, and cholangiocarcinoma.

Observation:

  • A proportion of patients present with established cirrhosis.
  • Liver transplantation is effective for various cholestatic liver diseases.
  • Transplantation for hepatolithiasis remains infrequently reported.

Findings:

  • This study reports on four patients with hepatolithiasis and secondary biliary cirrhosis who underwent liver transplantation.
  • All four patients experienced successful outcomes following the procedure.

Implications:

  • Liver transplantation is a viable and successful treatment for end-stage liver disease caused by hepatolithiasis.
  • These findings may encourage wider consideration of transplantation for select hepatolithiasis patients.
  • Further research into long-term outcomes is warranted.
