Summary

Recurrent pyogenic cholangitis (RPC) treatment remains challenging. Hand-assisted laparoscopic segmentectomy offers a potentially curative option with the lowest recurrence rates for intrahepatic stones.

Area of Science:

  • Hepatobiliary Surgery
  • Minimally Invasive Procedures
  • Gastroenterology

Background:

  • Recurrent pyogenic cholangitis (RPC) is common in Southeast Asia.
  • Pigmented intrahepatic stones are difficult to clear endoscopically or percutaneously.
  • Existing treatments have high recurrence rates.

Purpose of the Study:

  • To review the efficacy of hand-assisted laparoscopic segmentectomy (HALS) for RPC.
  • To evaluate the recurrence rates and complications associated with HALS in RPC patients.
  • To assess the feasibility and safety of HALS for treating intrahepatic stones.

Main Methods:

  • Single-center review of patients with RPC treated with HALS.
  • Analysis of operative outcomes, including stone clearance and recurrence.

Related Experiment Videos

  • Assessment of complication rates and long-term follow-up.
  • Main Results:

    • HALS demonstrated a low recurrence rate for RPC.
    • The procedure was feasible and safe in the studied patient cohort.
    • Complication rates were comparable to other surgical approaches, considering disease severity.

    Conclusions:

    • Segmentectomy, particularly HALS, is an effective treatment for RPC with intrahepatic stones.
    • This approach offers the potential for a definitive cure with reduced recurrence.
    • Laparoscopic liver resection is a viable option for managing complex RPC cases.
