Hepatocellular carcinoma (HCC) rarely presents with bone metastasis. This study details two rare cases, highlighting the importance of considering HCC in bone metastasis differential diagnoses.

Background:

  • Hepatocellular carcinoma (HCC) is a primary liver malignancy.
  • Bone metastasis is an uncommon complication of HCC, typically occurring after treatment of the primary tumor.

Observation:

  • Two cases of HCC presenting with bone metastasis at initial diagnosis are reported.
  • One patient had a rib metastasis, while the other presented with spinal cord compression due to a vertebral metastasis.

Findings:

  • Metastatic HCC to bone at initial presentation is extremely rare.
  • Multimodality treatment, including surgery and intra-arterial chemotherapy, offered palliative benefits but had limited impact on survival due to micrometastasis dissemination and comorbidities.
