Summary

Transient visual disturbances (TVDs) can occur during migraine without aura (MO) attacks. Current diagnostic criteria may overestimate migraine with aura (MA) diagnoses when TVDs are present in MO.

Background:

  • Migraine without aura (MO) is a common neurological disorder.
  • Transient visual disturbances (TVDs) are typically associated with migraine with aura (MA).
  • The International Headache Society (IHS) classification criteria differentiate between MO and MA based on the presence of aura symptoms.

Purpose of the Study:

  • To determine the frequency and characteristics of TVDs during MO attacks.
  • To compare these TVDs with the diagnostic criteria for MA.

Main Methods:

  • A cohort of 191 patients diagnosed with MO was studied.
  • Patients were referred to a specialized headache center.
  • Data on the occurrence and nature of TVDs during MO attacks were collected.

  • 13.6% of MO patients reported experiencing TVDs during their attacks.
  • TVDs were brief, often lasting less than 30 seconds.
  • Common TVDs included phosphenes, blurred vision, and scintillating scotomas.
  • If strict IHS criteria were applied, 46% of MO patients with TVDs could have been misdiagnosed with MA.

Conclusions:

  • TVDs can occur in a subset of patients with MO.
  • Current IHS diagnostic criteria may lead to an overdiagnosis of MA.
  • Further refinement of diagnostic criteria may be needed to accurately distinguish between MO and MA when visual disturbances are present.
