Background:

Lumbar sympathetic blocks are utilized for managing various pain conditions.
Traditional techniques may be associated with pain, paresthesias, and anesthesia.
A simplified and improved approach is needed.

Purpose of the Study:

To describe and evaluate a single-needle lateral approach for lumbar sympathetic block.
To compare its efficacy and safety with the classic technique.

Main Methods:

A single-needle lateral approach was employed in prone patients.
A 22-gauge, 14-cm needle was inserted 12.5 cm lateral to the L4 spinous process at a 35-40 degree angle.
After positioning opposite the anterolateral vertebral body, a 20 ml local anesthetic was injected with the patient in slight Trendelenburg position.

Main Results:

The anesthetic solution flowed cephalad, covering the sympathetic chain up to L-2.
This method proved to be less painful than the classic technique.
Reduced incidence of paresthesias and inadvertent sensory anesthesia was observed.
The procedure was noted to be easier to perform.

Conclusions:

The single-needle lateral approach is a safe and effective modification for lumbar sympathetic blocks.
It offers significant advantages in terms of patient comfort and procedural simplicity.
This technique represents an improvement over the classic lumbar sympathetic block procedure.
