Background:

  • Proximal ureteral strictures, particularly those involving the ureteropelvic junction, present significant reconstructive challenges.
  • Traditional repair methods may be limited by the extent of the defect and surrounding tissue viability.

Observation:

  • A 37-year-old male with a 6 cm proximal ureteral stricture secondary to trauma required surgical intervention.
  • Primary pyeloureterostomy was not feasible due to the nature of the stricture and patient anatomy.

Findings:

  • The appendix was successfully utilized as an autologous substitute to bridge the 6 cm ureteral defect, creating an isoperistaltic anastomosis from the renal pelvis to the proximal ureter.
  • Postoperative evaluations including retrograde pyelography, ureteroscopy, excretory urography, and mercaptoacetyltriglycine-3 renal scan confirmed a patent anastomosis, viable appendix, and no evidence of obstruction at 6 months.
