Laparoscopic donor nephrectomy (LapDN) is more cost-effective than open donor nephrectomy (OpenDN), offering better quality of life for donors at a lower overall cost. This makes LapDN a dominant strategy in kidney donation.

Area of Science:

  • Nephrology
  • Surgical Innovation
  • Health Economics

Background:

  • Laparoscopic donor nephrectomy (LapDN) offers faster recovery for donors compared to open donor nephrectomy (OpenDN).
  • Recipient graft function is equivalent between LapDN and OpenDN.
  • LapDN has historically incurred higher costs than OpenDN.

Purpose of the Study:

  • To compare the cost-utility of LapDN versus OpenDN.
  • To determine if LapDN is a cost-effective alternative to OpenDN from a societal perspective.

Main Methods:

  • Cost-utility analysis using time trade-off for utilities.
  • Probabilities derived from a systematic literature review.
  • Costs analyzed from 27 OpenDN and 34 LapDN patients.
  • Inclusion of lost employment costs.

Related Experiment Videos

  • Calculation of incremental cost-effectiveness ratio (ICER) with sensitivity analyses.
  • Main Results:

    • LapDN demonstrated lower costs ($11,170.71) compared to OpenDN ($12,631.91).
    • LapDN yielded superior quality of life (0.7247 QALYs) versus OpenDN (0.6585 QALYs).
    • LapDN was found to be a dominant strategy, remaining cost-effective even in worst-case scenarios (ICER $2,231.61/QALY).

    Conclusions:

    • LapDN provides improved quality of life and lower costs compared to OpenDN.
    • Cost-effectiveness was demonstrated even with data from the LapDN learning curve.
    • LapDN may increase organ donor rates and reduce costs associated with dialysis.
