Summary

This case study highlights psoriatic onycho-pachydermo-periostitis, a rare form of psoriatic arthritis. Methotrexate therapy effectively treated the patient's unusual psoriatic arthritis symptoms.

  • Psoriatic arthritis (PsA) affects joints in some individuals with psoriasis.
  • Palmar and nail involvement are common in psoriasis.

Observation:

  • A patient with a 2-year history of palmar psoriasis and nail involvement developed psoriatic onychodystrophy.
  • Painful swelling of digits and acral lamellar periostitis on X-ray were observed.
  • This presentation was consistent with a rare variant of psoriatic arthritis.

Findings:

  • The patient was diagnosed with psoriatic onycho-pachydermo-periostitis.
  • A 6-week course of methotrexate (15mg/week) led to normalized nail growth.
