Summary

Psoriatic onycho-pachydermo-periostitis (POPP) is a rare psoriatic arthritis complication. This case study found sulfasalazine and radiotherapy ineffective for symptomatic POPP.

Background:

  • Psoriatic onycho-pachydermo-periostitis (POPP) is a rare manifestation of psoriatic arthritis.
  • POPP presents as a triad of psoriatic onychosis, soft tissue thickening, and distal phalangeal osteoperiostitis, excluding distal interphalangeal arthritis.
  • Current treatment recommendations for symptomatic POPP are lacking.

Observation:

  • A 49-year-old male with symptomatic POPP was treated.
  • Treatment involved sulfasalazine (1000mg b.i.d. for 8 months) followed by radiotherapy to both hands (6x0.5 Gy).

Findings:

  • Neither sulfasalazine nor radiotherapy resulted in any observed clinical response.
  • This single case suggests these treatments are not effective for POPP.

Implications:

  • The findings indicate a need for further research into effective POPP treatments.
  • This case highlights the limited therapeutic options for this rare condition.
