Summary

Primary intracranial germinomas are rare, especially in the posterior fossa. This case highlights a cerebellar hemisphere germinoma in an adult, emphasizing the need for comprehensive evaluation of rare brain tumors.

Area of Science:

  • Neuro-oncology
  • Neuropathology

Background:

  • Germinoma cell tumors are rare primary cerebral tumors, typically found in the pineal and supra-sellar regions.
  • Primary germinomas originating in the posterior fossa, such as the cerebellar hemisphere, are exceptionally uncommon.

Observation:

  • A 45-year-old immunocompetent male presented with symptoms of intracranial hypertension and cerebellar ataxia.
  • CT imaging revealed a heterogeneous mass with a cystic component in the right cerebellar hemisphere, showing intense enhancement post-contrast.
  • Additional nodules were noted in the frontal and occipital lobes, while the pineal region appeared normal.

Findings:

  • Histopathological examination confirmed the diagnosis of germinoma after total resection of the cerebellar mass.
  • Systemic evaluation ruled out metastasis from an extracranial primary tumor, confirming the intracranial lesion as primary.

Related Experiment Videos

Implications:

  • This case underscores the possibility of primary intracranial germinomas occurring in rare locations like the cerebellar hemisphere in adults.
  • It highlights the importance of considering germinoma in the differential diagnosis of heterogeneous cerebellar masses, even in immunocompetent adults.
  • Comprehensive diagnostic workup is crucial to establish the primary nature of intracranial tumors and guide appropriate treatment strategies.
