Granular parakeratosis (GP) is a skin condition typically seen in adult women. This study identifies GP in young children, presenting as asymptomatic, hyperpigmented papules, challenging previous understandings of the disorder.

Background:

  • Granular parakeratosis (GP) is a rare skin condition characterized by hyperkeratotic papules and plaques in cutaneous folds.
  • Previously, GP was documented in only 24 adults, predominantly women over 40, with symptoms including pruritus.
  • Histopathology reveals a distinctive granular appearance in the parakeratotic horny layer.

Observation:

  • This study reports four cases of granular parakeratosis in children aged 10-24 months.
  • Affected children presented with asymptomatic, hyperpigmented, scaling papules on the groin, lower back, buttocks, and flanks.
  • Maternal history in all cases noted frequent washing and application of multiple topical products.

  • Histopathology in all pediatric cases confirmed the characteristic features of granular parakeratosis.
