Summary

Adding the aldosterone antagonist canrenone to standard treatment significantly improved left ventricular diastolic function in patients with essential hypertension. This benefit occurred independently of blood pressure changes or reductions in left ventricular mass.

Area of Science:

  • Cardiology
  • Pharmacology

Background:

  • Mineralocorticoid antagonists are known to prevent or reverse myocardial fibrosis.
  • Essential hypertension can lead to left ventricular diastolic dysfunction.

Purpose of the Study:

  • To test if the aldosterone antagonist canrenone improves left ventricular diastolic function in essential hypertension.
  • To investigate the direct myocardial effects of canrenone.

Main Methods:

  • A prospective, randomized, controlled study involving 34 never-treated essential hypertensives with diastolic dysfunction.
  • Utilized M-mode echocardiography and 24-hour ambulatory blood pressure monitoring (ABPM).
  • Patients received ACE inhibitors and calcium antagonists, with some randomly assigned to add canrenone.

Related Experiment Videos

Main Results:

  • Antihypertensive treatment reduced blood pressure, left ventricular mass index, and improved diastolic function.
  • The canrenone group showed a significantly greater increase in left ventricular diastolic indices compared to the control group.
  • This improvement was independent of changes in blood pressure and left ventricular mass.

Conclusions:

  • Low-dose aldosterone antagonist therapy, when added to existing antihypertensive treatment, significantly improves left ventricular diastolic function in essential hypertension.
  • The observed improvement in diastolic function is likely due to a direct effect of canrenone on the myocardium.
  • Canrenone represents a potential therapeutic option for managing diastolic dysfunction in hypertensive patients.
