Area of Science:

  • Dermatology
  • Infectious Diseases
  • Pulmonology

Background:

  • Cutaneous tuberculosis is a rare manifestation of Mycobacterium tuberculosis infection.
  • It can present polymorphously and may indicate visceral involvement.
  • Lupus vulgaris is the most common form in industrialized nations but remains rare.

Observation:

  • A 15-year-old female presented with asymptomatic, nodular, tumoral cutaneous lesions on her face and back.
  • Biopsy revealed non-caseating epithelioid granulomas with giant cells, and culture identified Mycobacterium tuberculosis.
  • The patient had no initial respiratory symptoms.

Findings:

  • Cavitating pulmonary tuberculosis was diagnosed via CT scan.
  • Acid-fast bacilli were detected in sputum.

Related Experiment Videos

  • The cutaneous lesions resolved completely following anti-tuberculosis treatment.
  • Implications:

    • Cutaneous lesions can be an unusual presenting feature of pulmonary tuberculosis.
    • Early recognition and treatment of cutaneous tuberculosis are crucial for managing potential systemic infection.
    • This case highlights the importance of investigating skin manifestations for underlying visceral disease.
