A rare case of subacute thyroiditis occurred in a woman with a history of Graves' disease. This highlights the complex interplay of autoimmune thyroid conditions.

Background:

  • Graves' disease is an autoimmune disorder causing hyperthyroidism.
  • Subacute thyroiditis is an inflammatory thyroid condition, often viral or post-viral.
  • Co-occurrence of these two distinct thyroid diseases in a single patient is uncommon.

Observation:

  • A 60-year-old woman presented with painful thyroid swelling and transient thyrotoxicosis.
  • Previous history of Graves' disease 30 years prior.
  • Current episode showed negative thyrotropin-receptor antibodies, decreased Tc-99m uptake (later normalized), and thyroid leucocyte infiltration on fine needle aspiration.

Findings:

  • The patient developed hypothyroidism three months after the initial presentation, requiring levothyroxine therapy.
