Postoperative psychosis after temporal resection for epilepsy is rare. Patients experiencing seizure recurrence on the opposite side of surgery are at higher risk for this psychiatric complication.

Area of Science:

  • Neurology
  • Neurosurgery
  • Psychiatry

Background:

  • Intractable epilepsy often necessitates surgical intervention, such as temporal resection.
  • Postoperative complications require careful monitoring and understanding.

Observation:

  • Three out of 282 patients developed postictal psychosis following temporal resections for epilepsy.
  • These patients experienced seizure recurrence contralateral to the surgical site.

Findings:

  • Contralateral seizure recurrence post-temporal resection is associated with the development of de novo postoperative postictal psychosis.
  • No psychiatric symptoms were observed in patients with ipsilateral seizure recurrence.

Implications:

  • Postoperative psychosis is a potential complication of temporal lobe epilepsy surgery.
