Paraquat poisoning in man.

J M Douze, A N van Heyst, A van Dijk

    Archives of Toxicology
    |October 20, 1975

    Summary

Paraquat dichloride poisoning requires immediate hemodialysis. Spectrophotometry and ion exchange chromatography help measure paraquat levels in bodily fluids, revealing low absorption rates.

    Area of Science:

    • Toxicology
    • Analytical Chemistry
    • Clinical Chemistry

    Background:

    • Paraquat dichloride (Gramoxone) is a herbicide with potential for severe human intoxication.
    • Accurate quantification of paraquat in biological samples is crucial for patient management and understanding its toxicokinetics.
    • Ion exchange chromatography is a viable method for sample clean-up prior to spectrophotometric analysis.

    Observation:

    • Three cases of Gramoxone intoxication were analyzed.
    • Paraquat dichloride concentrations were measured in blood, dialysate, feces, and urine.
    • Spectrophotometry was employed after sample purification using ion exchange chromatography (Dowex 50W-X12 or Zeo-Karb 225).

    Findings:

    • Zeo-Karb 225 was preferred over Dowex 50W-X12 for ion exchange chromatography, especially for large sample volumes.
    • Only 5-10% of an ingested paraquat dichloride dose is absorbed in humans.
    • Fullers' earth demonstrated significant utility in managing paraquat poisoning.

    Implications:

    • Early and primary hemodialysis is essential for treating paraquat dichloride intoxication.
    • Understanding paraquat absorption kinetics aids in clinical decision-making.
    • Refined analytical methods improve the diagnosis and monitoring of herbicide poisoning.
