Summary

This study reports a rare ectopic clival craniopharyngioma, a tumor separate from the sella turcica. An endonasal endoscopic approach successfully removed the lesion with minimal invasiveness.

Background:

  • Craniopharyngiomas are typically located in the sellar region, arising from remnants of Rathke's pouch.
  • Ectopic craniopharyngiomas, particularly those in the clival region and separate from the sella turcica, are exceptionally rare.

Observation:

  • A 44-year-old woman presented with abducens palsy, indicating neurological compromise.
  • Imaging (MRI and CT) revealed a cystic clival lesion distinct from the sella turcica.

Findings:

  • Surgical resection was successfully achieved using a flexible endoscope-assisted endonasal transsphenoidal approach.
  • Radiological, intra-operative, and clinical assessments confirmed no involvement of the sellar region.
  • A literature review identified no similar reported cases of ectopic clival craniopharyngioma.

Implications:

  • This case highlights the possibility of ectopic craniopharyngiomas arising from aberrant Rathke's pouch remnants.
  • The endonasal transsphenoidal endoscopic approach offers a minimally invasive and effective surgical option for select clival lesions.
  • Further research into the embryological origins and optimal surgical management of ectopic craniopharyngiomas is warranted.
