Area of Science:

  • Orthopedic Surgery
  • Biomaterials Science

Background:

  • Revision total knee replacement (TKR) procedures are increasing annually.
  • Existing literature often focuses on titanium-based revision systems.
  • A contemporary revision knee system with alternative design features warrants investigation.

Purpose of the Study:

  • To evaluate the mid-term clinical outcomes of a modular all-cobalt chrome stemmed revision knee system.
  • To assess the efficacy of hybrid stem fixation in revision TKR.

Main Methods:

  • Prospective study of 91 consecutive patients undergoing aseptic revision TKR.
  • Utilized a modular all-cobalt chrome stemmed revision knee system with hybrid stem fixation.
  • Follow-up included clinical scores (pain, function, range of motion, Knee Society score) and survivorship analysis.

Main Results:

  • Eighty-nine revisions in 84 patients were reviewed at a mean of 5.9 years.
  • Significant improvements observed in pain scores, function, range of motion, and total Knee Society score.
  • Kaplan-Meier survivorship was 93.5% at 8.6 years, with 5 patients requiring revision surgery.

Conclusions:

  • Intermediate results support the use of cobalt chrome components and hybrid cement fixation in revision TKR.
  • This contemporary system demonstrates favorable outcomes in terms of pain relief and functional improvement.
  • Continued monitoring is advised for hybrid fixation combined with high articular constraint in revision knee systems.
