    Background:

    • Polycythemia vera (PV) is a myeloproliferative neoplasm.
    • Transformation to acute leukemia is a known complication of PV.

    Purpose of the Study:

    • To investigate the preleukemic phase preceding acute leukemia in PV patients.
    • To identify early indicators of leukemic transformation in PV.

    Main Methods:

    • Prospective follow-up of 76 polycythemia vera patients.
    • Regular bone marrow examinations.
    • Autopsy findings analysis.

    Main Results:

    • Six patients developed acute leukemia after a preleukemic phase.
    • Preleukemic phase characterized by pancytopenia and megaloblastoid erythropoiesis with ring sideroblasts.
