Background:

  • Cerebral small-vessel disease (SVD) is a significant cause of stroke and cognitive decline.
  • Retinal vascular changes are increasingly recognized as potential indicators of systemic vascular health.

Purpose of the Study:

  • To investigate the association between retinal artery disease and cerebral small-vessel disease (SVD).

Main Methods:

  • Prospective cohort study involving 179 patients with symptomatic atherosclerotic disease.
  • Retinal photographs and brain MRI were analyzed for vascular abnormalities.
  • Standardized scoring lists were used for assessing retinal arterial narrowing, crossings, sclerosis, and tortuosity.
  • White matter lesions (WML) and lacunar infarcts were assessed via MRI.

  • 92% of patients with SVD showed at least one retinal vascular abnormality, compared to 77% without SVD (p < 0.01).

Conclusions:

  • Pathological changes in retinal arteries parallel those in small cerebral arteries.
