Summary

Ictal magnetoencephalography (MEG) shows promise for epilepsy surgery evaluation. This study found ictal MEG provided useful seizure localization information, sometimes superior to invasive electroencephalography (EEG).

Background:

  • Epilepsy surgery requires precise seizure focus localization.
  • Magnetoencephalography (MEG) is a non-invasive neuroimaging technique.

Observation:

  • This study investigated the feasibility of ictal (seizure) MEG recordings.
  • Twenty patients with epilepsy underwent MEG during seizures.

Findings:

  • Successful ictal MEG recordings were achieved in 6 of 20 patients with neocortical epilepsy.
  • Ictal MEG provided seizure onset localization information comparable or superior to invasive electroencephalography (EEG) in most successful cases.
  • Interictal MEG was less informative than ictal MEG in some patients.

Implications:

  • Ictal MEG may be a valuable tool in presurgical epilepsy evaluation.
  • Further research with larger cohorts is needed to confirm these findings.
  • This technique could improve surgical outcomes for epilepsy patients.
