Summary

Epilepsy linked to hypothalamic hamartomas originates deep within the brain near the hamartoma, not the cortex. This finding challenges previous assumptions about seizure origins in this condition.

Background:

Observation:

  • This study analyzed interictal spikes in 4 patients (ages 2-25) with epilepsy and hypothalamic hamartoma using EEG and MRI-based models.
  • Multifocal spike activity, primarily in frontal and temporal lobes, was observed.
  • Source analysis employed single moving dipole models and the Rap-MUSIC algorithm.

Findings:

  • EEG spikes were best explained by deep sources near the hamartoma, followed by later cortical involvement.
