    Background:

    • Primary adenocarcinoma of the appendix is an exceptionally rare malignancy.
    • Fewer than 200 cases have been documented, necessitating further clinical investigation.

    Purpose of the Study:

    • To analyze treatment strategies and prognoses for primary appendiceal adenocarcinoma.
    • To compare the efficacy of appendectomy versus right hemicolectomy for different subtypes.

    Main Methods:

    • Retrospective analysis of 20 appendiceal adenocarcinoma cases.
    • Review of 39 additional cases from existing literature.
    • Evaluation of patient demographics, symptoms, treatment modalities, and survival outcomes.

    Main Results:

    • For malignant mucocele, appendectomy alone yielded a 5-year survival rate of 85.7% (6/7 patients).
    • For colonic-type adenocarcinoma, right hemicolectomy showed a higher 5-year survival rate (60% in literature) compared to appendectomy alone (22% in this study, 46% in literature).

    Conclusions:

    • Appendectomy alone is a potentially curative option for malignant mucocele confined to the appendix tip without submucosal invasion.
    • Right hemicolectomy is the indicated treatment for colonic-type appendiceal adenocarcinoma to improve long-term survival.
