Summary
Treating kidney stones in a pelvic kidney is difficult. This study details a new minimally invasive technique using laparoscopy-assisted percutaneous nephrolithotomy to access stones in the lower calix of a pelvic kidney.
Area of Science:
- Urology
- Minimally Invasive Surgery
Background:
- Urolithiasis, or kidney stones, in pelvic kidneys poses unique surgical challenges due to anatomical location.
- Percutaneous nephrolithotomy (PCNL) is a standard treatment for kidney stones, but its application in pelvic kidneys requires specialized approaches.
Observation:
- A novel extraperitoneal laparoscopy-assisted technique was developed to facilitate access to the lower-pole calix of a pelvic kidney.
- This approach combines laparoscopic visualization with percutaneous access for stone removal.
Findings:
- The described technique successfully enabled percutaneous nephrolithotomy in a patient with a pelvic kidney stone.
- This method provides a viable option for managing complex urolithiasis in challenging anatomical locations.
Implications:
- This approach may offer a less invasive alternative for treating kidney stones in pelvic kidneys.
- Further studies are warranted to evaluate the efficacy and safety of this technique in a larger patient cohort.

Urinary Tract Calculi VI: Surgical Management
Procedures for Kidney StonesMedical intervention is necessary when kidney stones or renal calculi are too large to pass spontaneously (typically greater than 5 millimeters) when stones are accompanied by symptomatic infection (such as fever or pyelonephritis), when they impair kidney function, or when they cause persistent symptoms like severe pain, nausea, or urinary retention. Additionally, patients with only one kidney or those who cannot be treated with medical management also require...
Kidney Transplant II: Surgical Procedure
Preoperative ManagementThe primary goals of preoperative management in kidney transplantation are to optimize the patient’s metabolic state and prepare them for surgery through diet adjustments, necessary dialysis, and tailored medical treatment. This phase also involves comprehensive infection screening and patient education about the surgical procedure and postoperative care to improve outcomes and adherence.Medical ManagementA comprehensive evaluation is required for both the living donor...
