A rare complication of peribulbar anesthesia during pupilloplasty led to brainstem anesthesia, necessitating mechanical ventilation. While the patient recovered well, this case highlights the risks associated with peribulbar anesthesia.

Background:

Peribulbar anesthesia is commonly used for ophthalmic procedures.
Complications, though rare, can be severe.

Observation:

A patient undergoing pupilloplasty under peribulbar anesthesia developed brainstem anesthesia.
This required tracheal intubation and mechanical ventilation.

Findings:

The most likely cause was an accidental subarachnoid (intrathecal) injection of local anesthetic.
