Area of Science:

  • Urology
  • Oncology
  • Diagnostic Imaging

Context:

  • Standard sextant biopsies for prostate cancer may be insufficient.
  • Increasing biopsy numbers causes patient discomfort and higher costs.
  • A cost-effective strategy involves repeating biopsies selectively in high-risk cases.

Purpose:

  • To evaluate the efficacy and cost-effectiveness of repeating prostate biopsies in selected high-risk patients.
  • To compare detection rates between standard and selective repeat biopsy strategies.
  • To assess the economic impact of a risk-stratified repeat biopsy protocol.

Summary:

  • A modified sextant technique (6 biopsies) was used for initial prostate biopsies.
  • Biopsies were repeated within 4 months for 11 patients with high-risk clinical parameters (PSA > 10 or nodule) and 25 with PSA velocity increase (> 0.75 ng/year).

Related Experiment Videos

  • Repeat biopsies in high-risk and PSA velocity change groups yielded positive results in 3/11 and 9/25 patients, respectively.
  • Impact:

    • Selective repeat biopsies in high-risk cases can reduce false negatives compared to increased standard biopsies.
    • This strategy significantly reduces the number of unnecessary biopsies, saving costs and patient discomfort.
    • The study demonstrated substantial economic savings for the healthcare system by avoiding non-indicated histological examinations.
