Area of Science:

  • Vascular Medicine
  • Urology
  • Rheumatology

Background:

  • Erectile dysfunction (ED) in young men often requires thorough investigation beyond common causes.
  • Hypertension can be associated with vascular complications affecting sexual health.

Observation:

  • A 30-year-old male presented with persistent erectile dysfunction since age 26.
  • He was diagnosed with nodal polyarteritis, a systemic vasculitis.
  • Initial investigations for ED included penile brachial index (PBI) and papaverine hydrochloride infusion.

Findings:

  • The patient exhibited arterial erectile dysfunction, indicated by PBI measurements and response to papaverine.
  • Internal-pudendal angiography revealed multiple microaneurysms in bilateral internal pudendal arteries.

Related Experiment Videos

  • These findings strongly suggest nodal polyarteritis as the cause of his arterial erectile dysfunction.
  • Implications:

    • Nodal polyarteritis should be considered in the differential diagnosis of vasculitic-induced erectile dysfunction in young males.
    • Early diagnosis and management of vasculitis may prevent irreversible vascular damage and improve erectile function.
    • This case underscores the link between systemic inflammatory conditions and sexual dysfunction, necessitating a multidisciplinary approach.
