    Background:

    • Lithium is a common maintenance treatment for bipolar disorder.
    • Weight gain is a frequently reported side effect of lithium therapy.

    Purpose of the Study:

    • To investigate the prevalence and contributing factors of weight gain in patients undergoing long-term lithium maintenance treatment.

    Main Methods:

    • Retrospective review of case records for 70 patients with bipolar disorder on lithium for 2-10 years.
    • Patient questionnaires assessing weight changes, appetite, thirst, and fluid intake.

    Main Results:

    • 45 out of 70 patients (64%) experienced weight gain, averaging 10 kg.
    • Weight gain correlated strongly with increased fluid intake and thirst, not appetite.
    • Patients gaining weight were already overweight and exceeded ideal weight by approximately 20%.

    Conclusions:

    • Increased thirst and subsequent high-calorie fluid consumption are key drivers of lithium-associated weight gain.
    • Patients on lithium should be advised to avoid calorie-rich fluids to mitigate weight gain risks.
