Background:

  • Spermatoceles are common cystic masses of the epididymis.
  • Large or bilateral spermatoceles are less frequently encountered.
  • This report focuses on a rare case of exceptionally large, bilateral, multilocular spermatoceles.

Observation:

  • A 45-year-old male presented with bilateral, large inguinoscrotal masses present since adolescence.
  • Gradual size increase of the masses over the past 5 years was noted.
  • Scrotal ultrasonography revealed large, multilocular cystic masses measuring 65x45x50 mm (right) and 55x45x40 mm (left).

Findings:

  • Surgical excision via bilateral inguinoscrotal incision was performed, preserving the epididymal body.
  • Pathological correlation confirmed the diagnosis of spermatocele.
