Male incontinence: relationship of physiology to surgery

After prostatectomy it is the passive continence mechanism that maintains continence. Since this mechanism is less effective when the structures are not in the normal elevated position, perineal continence operations are probably successful in proportion to the mobility of the sub-prostatic structures and the ability to keep them elevated.
