Background:

Myelofibrosis is a bone marrow disorder characterized by fibrosis and extramedullary hematopoiesis.
Extramedullary hematopoiesis can occur in various organs, including the spleen, liver, and lymph nodes.
Spinal cord compression is a serious neurological emergency requiring prompt diagnosis and management.

Observation:

A patient presented with symptoms suggestive of spinal cord compression.
Imaging revealed a large pelvic mass.
The mass was identified as extramedullary hematopoietic tissue originating from myelofibrosis.

Findings:

The extramedullary hematopoietic tissue from the pelvic mass infiltrated the spinal canal.
This infiltration led to significant spinal cord compression.
