  • Milky pericardial drainage with high triglycerides (197 mg/dl) confirmed the diagnosis.

Findings:

  • Conservative management with nothing by mouth for 2 days and a subsequent low-fat diet effectively resolved chylopericardium.
  • Pericardial drainage volume and turbidity decreased significantly after dietary modification.

Implications:

  • Dietary management is crucial for treating postoperative chylopericardium.
