Background:

  • The emergency department (ED) at Louisiana State University implemented a novel airway management strategy.
  • This strategy involved four distinct diagnosis-based protocols for rapid sequence intubation (RSI).

Purpose of the Study:

  • To evaluate the utilization and outcomes associated with these RSI protocols.
  • To identify patient and protocol variables predictive of adverse outcomes during RSI.

Main Methods:

  • A 4-year prospective, observational study was conducted in an academic ED.
  • Data collected included patient demographics, protocol used, diagnostic group, intubation indication, and pre-intubation oxygen saturation.
  • Adverse outcomes were defined as hemodynamic changes, need for intervention, or >3 intubation attempts. Statistical analyses included univariate, logistic regression, and regression tree analysis.

  • A total of 1,320 patients underwent RSI. Protocol C was associated with fewer adverse outcomes compared to Protocol D (OR=0.2).

  • Regression tree analysis identified specific patient groups (adult trauma on protocols B/D, adult medical on protocol B) at higher risk for adverse outcomes (8.5% vs 2.4%).

  • Physicians can be alerted to higher-risk patient populations, enabling proactive management during RSI.
  • Specific patient subgroups, including adult trauma and certain medical cases, require heightened vigilance during RSI.
