    Background:

    • Intraoperative hemodilution is a technique used during extensive surgeries.
    • The choice of fluid for hemodilution can impact patient hemodynamics and organ function.

    Purpose of the Study:

    • To compare the effects of hemodilution with lactated Ringer's solution versus Plasmanate on hemodynamics, oxygen transport, and lung water.
    • To evaluate the compensatory mechanisms following intraoperative prebleeding and hemodilution.

    Main Methods:

    • Four patients undergoing extensive surgery underwent hemodilution with lactated Ringer's solution.
    • Results were compared to a previous study group hemodiluted with Plasmanate.
    • Hemodynamic parameters, oxygen transport, and lung water were monitored.

    Main Results:

    • Plasmanate group showed a greater increase in cardiac output (161%) compared to lactated Ringer's group (115%).
    • Systemic oxygen transport increased with Plasmanate (109.7%) but decreased with lactated Ringer's (80.0%).
    • Plasmanate reduced lung water (82%) and improved oxygen gradients (75%), while lactated Ringer's increased them (134% and 112%, respectively).

    Conclusions:

    • Hemodilution with Plasmanate offers superior outcomes in oxygen transport and lung water management compared to lactated Ringer's solution.
    • Both hemodilution strategies were well-tolerated in this patient cohort.
    • Plasmanate may be a more effective fluid for intraoperative hemodilution in complex surgical cases.
