Summary

A rare case of botryomycosis, a bacterial skin infection, was diagnosed in an AIDS patient. Prompt surgical excision and antimicrobial therapy successfully treated the painful skin ulcers and systemic symptoms.

Background:

  • Human Immunodeficiency Virus (HIV) infection can lead to opportunistic infections.
  • Botryomycosis is a rare chronic bacterial infection often presenting with skin lesions.

Observation:

  • A 28-year-old male with Acquired Immunodeficiency Syndrome (AIDS) presented with generalized, painful skin ulcers, fever, and malaise.
  • Differential diagnoses included viral infections and other fungal or bacterial granulomas.
  • Histopathology showed gram-positive coccoid bacteria in the dermis with inflammation.

Findings:

  • Bacterial culture confirmed Staphylococcus aureus as the causative agent.
  • Viral cultures were negative, ruling out common viral etiologies.
  • The patient was diagnosed with botryomycosis.

Implications:

  • Surgical excision of large lesions combined with appropriate antimicrobial therapy led to complete resolution of symptoms.
  • This case highlights the importance of considering botryomycosis in immunocompromised patients with atypical skin infections.
  • Further research into botryomycosis in the context of HIV infection is warranted.
