    A rare ameloblastic sarcoma in a young male showed complete response to chemotherapy. The patient later developed an aggressive malignant melanoma.

    Background:

    • Ameloblastic fibroma is a rare benign odontogenic tumor.
    • Ameloblastic sarcoma represents a malignant transformation of ameloblastic fibroma.
    • Recurrent tumors pose significant treatment challenges, especially in pediatric patients.

    Observation:

    • A young male patient presented with a recurrent tumor.
    • The tumor demonstrated malignant progression from ameloblastic fibroma to ameloblastic sarcoma.
    • The patient received combination chemotherapy.

    Findings:

    • Chemotherapy with Actinomycin D, Vincristine, and Cytoxan resulted in a complete response.
    • No recurrence was observed for 4 years post-chemotherapy.
    • The patient subsequently developed an aggressive malignant melanoma 1 year after treatment cessation.

    Implications:

    • This case highlights the potential for ameloblastic fibroma to progress to sarcoma.
    • Effective chemotherapy regimens can achieve durable remission in ameloblastic sarcoma.
    • Long-term surveillance is crucial due to the risk of secondary malignancies.
