Summary

Orbital granulocytic sarcoma, a rare initial symptom of acute myelogenous leukemia (AML), can present without typical leukemic signs. Early detection and irradiation can improve vision, highlighting the importance of prompt diagnosis in myeloproliferative syndrome (MPS) cases.

Background:

  • Myeloproliferative syndromes (MPS) can rarely manifest with ocular symptoms.
  • Orbital granulocytic sarcoma is an uncommon extramedullary tumor associated with myeloid malignancies.

Observation:

  • A 73-year-old woman with a history of MPS presented with bilateral chemosis, conjunctival redness, burning, and severely impaired ocular motility.
  • Ophthalmological examination revealed markedly distended conjunctivas, and biopsy confirmed conjunctival granulocytic sarcoma.
  • Diagnosis of acute myelogenous leukemia (AML) was confirmed via peripheral blood smear and bone marrow aspiration.

Findings:

  • The orbital tumor resolved completely following local external beam irradiation (30 Gy), with no recurrence.
  • Chemotherapy after irradiation achieved transient hematological remission.
