Late-onset vasculopathy can develop months after meningitis treatment, even in adults. This case highlights progressive intracranial arterial stenosis as a delayed vascular complication following pyogenic meningitis.

Background:

  • Vasculopathy is a known complication of acute meningitis.
  • No prior reports exist of vasculopathy developing months after meningitis treatment.

Observation:

  • A 51-year-old woman presented with severe headache 2 months post-treatment for Haemophilus influenzae type C meningitis.
  • Arteriography revealed progressive multifocal intracranial stenosis, primarily affecting internal carotid arteries.

Findings:

  • Pathologic examination showed diffuse collagenosis, indicating chronic vascular injury from meningitis.
  • Arterial lesions stabilized, and the patient remained asymptomatic.

Implications:

  • Progressive intracranial arterial stenosis can manifest months after meningitis.
