Summary

Elderly patients taking risperidone and selective serotonin-reuptake inhibitors (SSRIs) may develop serotonin syndrome. This potentially fatal condition requires careful monitoring and differential diagnosis when agitation increases with escalating risperidone doses.

Background:

  • Selective serotonin-reuptake inhibitors (SSRIs) are commonly prescribed for depression in elderly patients.
  • Risperidone is frequently used to manage agitation in this population.

Observation:

  • Two cases of elderly patients developed serotonin syndrome while on combined therapy with risperidone and paroxetine (an SSRI).
  • Symptoms included confusion, agitation, weakness, tremor, and muscle incoordination, worsening with increased risperidone dosage.

Findings:

  • Serotonin syndrome was strongly suspected in both cases, with causality assessed as probable and definite.
  • One patient died, while the other recovered after discontinuation of both medications.
