    Cerebellitis, an uncommon symptom of infectious mononucleosis, is reported for the first time in Australia. Early diagnosis can be challenging, but patients typically make a full recovery.

    Background:

    • Infectious mononucleosis (IM) is a common viral illness.
    • Neurological manifestations of IM are rare.

    Observation:

    • A case of cerebellitis, an inflammation of the cerebellum, is presented.
    • This represents the first reported instance of cerebellitis associated with IM in Australia.

    Findings:

    • The patient experienced a complete recovery, consistent with previous reports.
    • Diagnostic challenges in the early stages of this condition are highlighted.

    Implications:

    • Screening for IM is recommended in cases of unexplained neurological symptoms.
    • The role of cerebrospinal fluid antibodies in diagnosing IM-related cerebellitis warrants further investigation.
