Summary

Endobronchial brachytherapy offers palliative relief for lung cancer symptoms in 60-80% of patients and achieves 60-70% local control for localized tumors. This treatment can be curative, especially when combined with external irradiation for early-stage lung cancer.

Area of Science:

  • Oncology
  • Radiation Oncology
  • Pulmonology

Background:

  • Endobronchial brachytherapy is a localized radiation technique used for lung cancer.
  • It involves inserting a catheter near endobronchial lesions to deliver targeted irradiation while sparing healthy tissues.

Purpose of the Study:

  • To review the indications, efficacy, and complications of endobronchial brachytherapy.
  • To evaluate its role in palliative, curative, and combined treatment settings for lung cancer.

Main Methods:

  • Flexible bronchoscopy with afterloading catheter insertion.
  • Review of clinical data for palliative and curative treatments.
  • Analysis of randomized trials combining external irradiation and brachytherapy.

Related Experiment Videos

Main Results:

  • Palliative treatment improved respiratory symptoms in 60-80% of lung carcinoma cases.
  • Curative treatment achieved 60-70% local control at 2 years for localized endobronchial carcinomas.
  • Combined therapy showed efficacy in early-stage lung cancers, but not advanced stages in randomized trials.
  • Major complications include hemoptysis and radiation bronchitis, with better understanding of predictive factors.

Conclusions:

  • Endobronchial brachytherapy is effective for palliation and local control of endobronchial lung tumors.
  • Combination with external irradiation is promising for early-stage disease.
  • Ongoing research aims to optimize technical factors to reduce treatment-related toxicities.
