Aneurysm rupture during angiography: does acute vasospasm occur?

    Summary

    This study reviewed 32 cases of ruptured aneurysms, finding no definite proof of immediate intracranial arterial spasm post-rupture. Vasospasm observed was likely related to prior bleeding, not acute events.

    Background:

    • Cerebral aneurysms can rupture, leading to subarachnoid hemorrhage.
    • Intracranial arterial spasm is a known complication, but its acute phase post-rupture is debated.
    • Angiography is crucial for diagnosing ruptured aneurysms and assessing complications.

    Observation:

    • Thirty-two cases of aneurysm rupture during angiography were analyzed.
    • In cases with significant bleeding (extravasation), arteries were obscured, hindering spasm assessment.
    • Arterial spasm was present in 13 patients, but often linked to prior bleeding events.

    Findings:

    • No definitive evidence of an immediate, acute phase of intracranial arterial spasm was found following aneurysm rupture in this series.
    • Observed vasospasm was more likely associated with previous bleeding episodes than the acute rupture event.
    • Eight cases showed no evidence of arterial spasm.

    Implications:

    • The findings challenge the concept of acute vasospasm as an immediate consequence of aneurysm rupture.
    • Further research is needed to clarify the timing and etiology of vasospasm after subarachnoid hemorrhage.
    • Understanding vasospasm mechanisms is critical for optimizing treatment strategies in cerebrovascular disease.
