Decreased vitamin D receptor (VDR) and calcium-sensing receptor (CaSR) expression in parathyroid adenomas correlates with increased cell proliferation and disease severity in primary hyperparathyroidism. These findings suggest a role for VDR and CaSR in regulating parathyroid cell growth.

Area of Science:

  • Endocrinology
  • Molecular Biology
  • Oncology

Background:

  • Parathyroid (PT) adenomas in primary hyperparathyroidism (1°HPT) exhibit reduced expression of the vitamin D receptor (VDR) and calcium-sensing receptor (CaSR).
  • The relationship between parathyroid gland proliferative activity and the expression levels of VDR and CaSR remains incompletely understood in the context of 1°HPT severity.

Purpose of the Study:

  • To investigate the correlation between parathyroid gland proliferative activity and VDR/CaSR protein expression in patients with 1°HPT.
  • To compare these relationships with the clinical severity of 1°HPT.

Main Methods:

  • Immunohistochemical analysis of parathyroid tissue from seven patients with 1°HPT and four control subjects using antibodies against CaSR, VDR, and Ki67.
  • Quantification of Ki67 labeling index (LI) as a measure of cell proliferation.

Related Experiment Videos

  • Semi-quantitative assessment of CaSR and VDR expression intensity.
  • Main Results:

    • PT adenomas showed significantly lower VDR and CaSR scores and higher Ki67 LI compared to normal PTGs (P<0.01).
    • Ki67 LI positively correlated with serum intact parathyroid hormone, serum calcium levels, and PTG weight (R=0.70-0.84, P<0.05).
    • Ki67 LI negatively correlated with CaSR and VDR expression scores (R=-0.72 to -0.78, P<0.05), with a strong positive correlation between CaSR and VDR expression (R=0.95, P<0.001).

    Conclusions:

    • Reduced expression of VDR and CaSR in parathyroid adenomas is associated with increased parathyroid cell proliferation.
    • These molecular changes may contribute to the pathological progression of primary hyperparathyroidism.
